Supported Living and Outreach Services

Independent Living 

Yarrow offers a broad and evolving spectrum of housing options, each reflecting the unique aspirations and needs of individual service users.

Choices range from independent flats - some with a next door neighbour who also has a learning disability - to houses or flats shared with others.

Yarrow supports each service user living independently with on-site visits, assisting them with everyday tasks, such as cooking, grocery shopping, collecting benefit forms to supporting them with their personal relationships. We believe in encouraging participation within the community to help develop confidence and ensure quality of life.

Short Breaks - Outreach Service 

Yarrow's Outreach Service looks at encouraging service users to access a range of valuable activities - both in and out of the home - and to build relationships in their community. We aim to provide the user with stimulating activity, while also allowing a break for both the carer and user.

Currently Yarrow works in partnership with a number of bed-based services in two West London Boroughs. Our multi-agency and flexible approach offers carers and service users a number of variations to the Short Breaks scheme.

The service is also available for those who need respite for limited periods - perhaps during holidays, in an emergency or after hospitalisation.

Family Support Service 

Carers are often isolated as a result of their caring role. As a result, they may have lacked the opportunity to develop their own social support networks. The Family Support Service fills this gap by providing a flexible and responsive service that incorporates both practical and emotional support.

Practical support includes supplying the carer with information and advice on relevant services, organisations and agencies - ranging from college courses to charities - and initiating contact where necessary. The service also encompasses accompanying carers to reviews and on visits to potential residential homes and respite placements.

Emotional support includes informal counselling, support in meetings with professionals and, where relevant, referral to other services. Yarrow's experiences show that regular contact can be key in identifying potential crisis situations before they become reality.
